Abstract | : | Background Standard insulin resistance (IR) marker now available was HOMA-IR which is expensive and not available widely as routine test so surrogated marker was needed. Adiposity excess, glucose and lipid metabolism marker were the three metabolic risk strongly correlated with IR. Epidemiology study found triglyceride glucose index (TyG), visceral adiposity index (VAI) dan lipid accumulation product (LAP) could predict HOMA-IR. Adding the 3rd metabolic risk such adiposity excess to TyG index gained discrimination value to predict IR. To the best of our knowledge this study never been held before in Javanese, Indonesia. Aims of this study was to investigated ability of the three index as single or combination to predict IR in Indonesia. Methods study design was cross sectional with 184 participants. IR describe as HOMA-IR more than 75 percentiles for each gender. Spearman correlation and receiver operating characteristic (ROC) was construct to recognized ability of the metabolic index to predict IR. Standard mathematical equation was used to calculated HOMA-IR, TyG, VAI and LAP. Addition of excess of adiposity marker to TyG, add glucose to VAI and LAP as multiply function was used to investigate the triple metabolic risk simultaneously. Result Combine index have higher correlation and AUC than single index. Value of LAP have higher correlation and AUC than TyG and VAI. Combination of TyG index have higher correlation than other combination indeks. The acceptable discrimination index signed by AUC > 0.7 to predict IR reach by TyG*BMI, TyG*WC, TyG* WtHR, TyG*VF and LAP* glucose. The highest AUC reach by TyG*WtHR. Conclusion we suggest TyG with combine of excess adiposity and LAP*glucose mention above could be used as surrogated marker to recognize IR in Javanese, Indonesia. This combination index acceptable but not as good as HOMA-IR to predict IR. |
